  • Maryland SCORE Community Health Worker

    The SCORE (Sickle Cell Outreach Resources and Engagement) Program Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community services and other resources to adopt healthy behaviors, maintain or retain care. The SCORE CHW supports providers and social service agencies with an integrated approach to care management and community outreach.
